Sleep Medicine

Sleep medicine is a medical specialty, which is a subspecialty of internal medicine, devoted to the diagnosis and treatment of sleep disorders.

How much sleep do I need?

Adults need 7 or more hours of sleep per night for the best health and wellbeing.

How common is insomnia?

Insomnia is the most common sleep disorder. Approximately 5% of the population experience chronic insomnia, with 25% of these cases being primary insomnia. One third of adults experience some degree of insomnia in any given year, and 2–6% use medications to aid sleep.

How about sleep apnea?

It is estimated that 22 million Americans suffer from sleep apnea, with 80 percent of the cases of moderate and severe obstructive sleep apnea undiagnosed.

The other common sleep disorder is restless legs syndrome – how common is it?

5 million U.S. adults (about 2-3 percent) have moderate to severe RLS. 80 percent of people with RLS also have periodic limb movement of sleep (PLMS) Symptoms continue longer and become more frequent with age. So, sleep disorders are not only common but also affect a significant portion of the population.

Polysomnography records several body functions during sleep, including brain activity, eye movement, oxygen and carbon dioxide blood levels, heart rate and rhythm, breathing rate and rhythm, the flow of air through the mouth and nose, snoring, body muscle movements, and chest and belly movement.

There are two types of polysomnography performed at the sleep lab

  • Diagnsotic polysomnography
  • CPAP or BIPAP titration study

Multiple sleep latency test

(MSLT) The MSLT measures, by several nap opportunities in one day, how long it takes a person to fall asleep. It also determines whether REM sleep appears upon falling asleep.

It is usually performed immediately after an overnight study.

Maintenance of wakefulness test (MWT)

This test measures whether a person can stay awake during a time when she or he is normally awake.

Like the MSLT, the MWT is performed in a sleep diagnostic center over 4 – 5 nap periods.

A mean sleep onset latency of less than 10 minutes is suggestive of excessive daytime sleepiness. Home Sleep Test (HST) The Home Sleep Test also called an Out of Sleep Center Test (OCST) is used exclusively for the diagnosis of sleep apnea.

Home sleep study

Portable equipment is taken to home with the patient. The channels are usually limited to airflow, respiratory effort, and oximetry.
