Sleep Medicine

Sleep medicine is a medical specialty, which is a subspecialty of internal medicine, devoted to the diagnosis and treatment of sleep disorders.

Realizing the bidirectional relationship between sleep disorders and weight gain, most W8MD medical weight loss centers of America’s weight loss centers also offer a comprehensive sleep medicine program to help deal with both sleep disorders and metabolic disorders including obesity. How much sleep do I need?

Adults need 7 or more hours of sleep per night for the best health and wellbeing.

How common is insomnia?

Insomnia is the most common sleep disorder. Approximately 5% of the population experience chronic insomnia, with 25% of these cases being primary insomnia. One third of adults experience some degree of insomnia in any given year, and 2–6% use medications to aid sleep.

How about sleep apnea?

It is estimated that 22 million Americans suffer from sleep apnea, with 80 percent of the cases of moderate and severe obstructive sleep apnea undiagnosed.

The other common sleep disorder is restless legs syndrome – how common is it?

5 million U.S. adults (about 2-3 percent) have moderate to severe RLS. 80 percent of people with RLS also have periodic limb movement of sleep (PLMS) Symptoms continue longer and become more frequent with age. So, sleep disorders are not only common but also affect a significant portion of the population.

Polysomnogram

Polysomnography records several body functions during sleep, including brain activity, eye movement, oxygen and carbon dioxide blood levels, heart rate and rhythm, breathing rate and rhythm, the flow of air through the mouth and nose, snoring, body muscle movements, and chest and belly movement.

There are two types of polysomnography performed at the sleep lab

  • Diagnsotic polysomnography
  • CPAP or BIPAP titration study

Multiple sleep latency test

(MSLT) The MSLT measures, by several nap opportunities in one day, how long it takes a person to fall asleep. It also determines whether REM sleep appears upon falling asleep.

It is usually performed immediately after an overnight study.

Maintenance of wakefulness test (MWT)

This test measures whether a person can stay awake during a time when she or he is normally awake.

Like the MSLT, the MWT is performed in a sleep diagnostic center over 4 – 5 nap periods.

A mean sleep onset latency of less than 10 minutes is suggestive of excessive daytime sleepiness. Home Sleep Test (HST) The Home Sleep Test also called an Out of Sleep Center Test (OCST) is used exclusively for the diagnosis of sleep apnea.

Home sleep study

Portable equipment is taken to home with the patient. The channels are usually limited to airflow, respiratory effort, and oximetry.

Home sleep study

The traditional way of testing for some of the sleep disorders is changing. With the availability of the more convenient, less expensive and more affordable home sleep studies, the traditional in the lab sleep studies are getting extinct. Many sleep centers have closed down due to insurances mandating home sleep studies as opposed to in lab sleep studies.

What sleep conditions can be tested with home sleep study?

Home sleep studies are primarily used to rule out one of the most common sleep problem called sleep apnea.

What is obstructive sleep apnea?

The sleep apnea-hypopnea syndrome (SAHS) is a respiratory disorder characterized by frequent breathing cessations (apneas) or partial collapses (hypopneas) during sleep.

How is sleep apnea traditionally diagnosed?

The gold standard method for SAHS diagnosis is in-hospital, technician-attended nocturnal polysomnography (PSG) which is a labor-intensive, time-consuming, and relatively costly test, and limited availability, especially in remote areas. These drawbacks have led to large waiting lists, which delay diagnosis and treatment and limits its effectiveness as single diagnostic method for sleep apnea lead to the development of new home sleep studies which are proven to be as accurate and effective in diagnosing obstructive sleep apnea.
Frequently asked questions related to home sleep studies
  • How is home sleep study performed?

At W8MD weight loss, sleep and medspa centers, you come in to consult with one of our sleep physicians who will go over your sleep history, do a physical and determine if you need a home sleep study. If it was determined that you can benefit from a home sleep study, then the physician will order a home sleep study which will be performed at your home. The staff at W8MD will provide the instructions on how to use the equipment and how to return it back to the clinic after the sleep study so the staff can download the information for doctors to review. Once the sleep doctor reviews your sleep study, you will be asked to return for a result review visit.

  • What is the cost of home sleep study?

Although the cost varies from center to center, at W8MD, our self pay discounted home sleep study cost is only $300.00 per night, and we typically recommend a two night study. If you do the two night study, we do both nights for only $500.00 per night for self pay patients.

  • Does my insurance cover home sleep study?

If you have insurance, then they might cover the home sleep studies.

  • Where can I find a sleep clinic or center that offers home sleep studies?

Most W8MD centers offer home studies.

  • Are the home sleep studies as accurate to determine if I have sleep apnea?

Studies have shown that home sleep studies are atleast as good and reliable as in lab sleep studies when it comes to sleep apnea.

  • Can home sleep studies be used to diagnose other sleep disorders such as Narcolepsy?

No. Narcolepsy requires an in lab sleep study called Polysomnograph followed by an Multiple Sleep Latency Test(MSLT) to properly diagnose narcolepsy.

  • How long does it take to do a home sleep study and get the results?

At most W8MD centers, we can do the sleep studies and get the results within 1-2 weeks although some of the hospital based sleep centers can take up to 3 months to do the sleep studies.
