Leg cramps vs. restless legs syndrome — W8MD medical team and health resource

Overview

Nighttime leg symptoms are often mislabeled. A muscle cramp is a sudden, painful contraction with a hardened muscle. Restless legs syndrome is an urge to move with uncomfortable sensations that begin or worsen at rest, improve with movement and are worse in the evening.

Key points

Painful contraction

Cramps are focal and painful, commonly in the calf or foot.

Urge to move

RLS is defined by the urge and circadian pattern rather than a visible cramp.

Sleep movements

Periodic limb movements occur during sleep and require different interpretation.

Causes

Iron deficiency, pregnancy, kidney disease, neuropathy and medications can contribute to RLS; dehydration and muscle strain may contribute to cramps.

Clinical guidance

Evaluation includes symptom timing, neurologic and vascular signs, medication review and selected iron studies. Routine quinine is not recommended for nocturnal leg cramps because risks can be serious. Iron should not be taken indefinitely without appropriate testing.

Frequently asked questions

Does magnesium cure leg cramps?

Evidence is inconsistent outside specific deficiency or pregnancy contexts.

Can RLS happen during the day?

Yes, when sitting or resting, but it is usually worse later in the day.

Do I need a sleep study?

Not for every classic case; it may help when another sleep disorder or unusual movements are suspected.
