Weight loss can include lean tissue

Substantial weight loss—whether achieved through lifestyle treatment, medication or surgery—usually includes some loss of lean mass as well as fat. Lean mass is not identical to skeletal muscle, but excessive loss of muscle can reduce strength, physical function and resting energy needs. Older adults, people with low baseline muscle, very rapid loss or severe appetite suppression deserve particular attention.

GLP-1 medicines should not automatically be blamed for every change in lean mass. The clinically useful question is whether the patient is losing weight with adequate nutrition, progressive resistance activity and monitoring of strength and function.

Adequate protein

Resistance training

Train major muscle groups two or more days weekly when medically appropriate, progressing gradually.

Avoid excessive restriction

Faster is not always better. Severe calorie restriction plus profound appetite suppression can make adequate intake difficult.

Monitor function

Follow strength, walking ability, waist, weight trend and body composition—not scale weight alone.

  1. Establish baseline weight, waist, strength and—when available—body composition.
  2. Set a protein goal and identify foods the patient can tolerate even when appetite is low.
  3. Use resistance exercises such as sit-to-stand, rows, presses, squats or supervised alternatives.
  4. Include aerobic activity for cardiovascular health without allowing it to replace strength work.
  5. Review medication dose and rate of loss if weakness, dizziness or inability to meet nutrition goals develops.
  6. Reassess during maintenance, when rebuilding strength may require progressive training and adequate energy.

Who needs extra caution

  • Adults over 65 or anyone with frailty
  • History of falls, sarcopenia or prolonged inactivity
  • Very-low-calorie diets or rapid weight loss
  • Kidney disease requiring individualized protein intake
  • Neuromuscular disease, severe arthritis or disability
  • Persistent nausea, vomiting or food intolerance
