Weight loss medication eligibility and BMI requirements — W8MD medical team and health resource

Eligibility is more than a BMI number

FDA-approved chronic weight-management medications are generally labeled for adults with obesity, commonly BMI 30 kg/m² or higher, or overweight, commonly BMI 27 kg/m² or higher, plus at least one weight-related condition. Exact indications vary by product and age group. A clinician must also determine whether expected benefit outweighs contraindications, interactions and reproductive considerations.

BMI is a screening measure, not a direct measurement of body fat or individual health. Insurance criteria may be narrower than FDA labeling and may require a specific diagnosis, documented lifestyle program, previous therapy or step treatment.

Common qualifying conditions

Metabolic

Type 2 diabetes, prediabetes, dyslipidemia, metabolic syndrome or fatty liver disease.

Cardiovascular

Hypertension, established cardiovascular disease or other weight-related risk.

Sleep and breathing

Obstructive sleep apnea or obesity hypoventilation in appropriate clinical settings.

Mechanical and reproductive

Osteoarthritis, mobility limitations or PCOS may be relevant, depending on product labeling and plan rules.

Medical screening before prescribing

  • Weight history, previous treatment and realistic goals
  • Current medicines, supplements and allergies
  • Pregnancy, breastfeeding and pregnancy plans
  • Diabetes therapy and hypoglycemia risk
  • Pancreatitis, gallbladder, kidney and gastrointestinal history
  • Personal or family thyroid cancer/MEN2 history when relevant to labeling
  • Mental health, eating-disorder and substance-use screening
  • Ability to follow up and sustain treatment

Medical eligibility versus insurance approval

A clinician may determine that treatment is medically appropriate while an insurance plan excludes anti-obesity drugs or requires additional criteria. Prior authorization is a request for coverage, not a guarantee. Patients should ask whether the medication is on the formulary, whether obesity treatment is excluded, what documentation is required and what copay applies after approval.

How W8MD can help

W8MD Weight Loss, Sleep & MedSpa provides physician-supervised medical weight management in Brooklyn, New York and Northeast Philadelphia, with telehealth when clinically and legally appropriate. Care may include medical evaluation, nutrition planning, body-composition monitoring, prescription medication, sleep evaluation and long-term maintenance.

When an FDA-approved medication is medically appropriate and a plan provides coverage, W8MD can review benefits and help submit prior-authorization documentation. Coverage and approval are determined by the insurer; no medication or result is guaranteed.

W8MD calculates BMI, documents comorbidities, reviews previous treatment and selects among GLP-1, dual-agonist and established oral options. When coverage is available, the team can submit supporting prior-authorization information and respond to insurer requests.

Frequently asked questions

Can I qualify with BMI below 27?

Most chronic weight-management drug labels do not use BMI below 27 as the general adult threshold, though another medication indication may apply. A clinician must review the exact product.

Does sleep apnea count as a comorbidity?

It can be a weight-related condition. Zepbound also has a specific FDA-approved OSA indication for certain adults with obesity and moderate-to-severe OSA.

Does qualifying medically guarantee insurance coverage?

No. Coverage depends on the plan, formulary, exclusions and authorization rules.
