Sudden loss of muscle tone triggered by emotion

Cataplexy is a brief episode of muscle weakness triggered by strong emotion—often laughter, surprise, excitement or anger—while consciousness is preserved. It can affect the jaw, face, neck, knees or the whole body. Cataplexy is strongly associated with narcolepsy type 1, but collapse has many other possible causes and requires expert evaluation.

Recognizing a possible episode

Preserved awareness

The person is usually conscious and remembers the episode.

Emotion trigger

Laughter or another strong emotion often precedes weakness.

Brief duration

Episodes commonly last seconds to a few minutes.

Variable severity

A subtle facial droop or knee buckling can occur without full collapse.

Diagnostic evaluation

  • Detailed description from the patient and witnesses
  • Review of sleepiness, sleep paralysis and dream-like hallucinations
  • Overnight polysomnography followed by a multiple sleep latency test when appropriate
  • Medication, substance, seizure and cardiac-syncope assessment
  • Selected cerebrospinal-fluid hypocretin testing in special situations

Safety and treatment

Until diagnosis and control are established, patients should discuss driving, heights, swimming, cooking and machinery. Treatment may include narcolepsy-directed medication, adequate sleep scheduling and management of daytime sleepiness. Medication selection is individualized and may involve controlled substances or drugs with important interaction and pregnancy considerations.

Frequently asked questions

Is cataplexy a seizure?

Usually not, because awareness is preserved, but seizure and syncope must be considered.

Can cataplexy occur without narcolepsy?

It is unusual but possible; accurate diagnosis is essential.

Does a home sleep test diagnose narcolepsy?

No. Narcolepsy evaluation generally requires laboratory testing after clinical assessment.
