Type 2 diabetes — W8MD medical team and health resource

What is type 2 diabetes?

Type 2 diabetes occurs when the body does not use insulin effectively and pancreatic beta cells cannot produce enough insulin to maintain normal glucose. It can develop at any age and often progresses gradually without obvious symptoms.

Symptoms and risk factors

Possible symptoms

Thirst, frequent urination, fatigue, blurred vision, slow-healing sores, recurrent infections, tingling or unexplained weight loss.

Risk factors

Family history, prediabetes, gestational diabetes, PCOS, aging, physical inactivity and overweight or obesity.

Complications

Heart and kidney disease, stroke, eye disease, neuropathy, foot problems and dental disease.

No symptoms

Many people feel well at diagnosis, making risk-based screening important.

Diagnostic tests

Test Prediabetes Diabetes
A1c 5.7–6.4% 6.5% or higher
Fasting plasma glucose 100–125 mg/dL 126 mg/dL or higher
2-hour oral glucose tolerance 140–199 mg/dL 200 mg/dL or higher
Random plasma glucose Not used alone 200 mg/dL or higher with classic symptoms

Comprehensive treatment

Area Examples
Nutrition and activity Individualized eating pattern, reduced sitting, aerobic and resistance activity
Weight management Lifestyle treatment, anti-obesity medicine or metabolic surgery when appropriate
Glucose medication Metformin, GLP-1/GIP therapies, SGLT2 inhibitors, insulin and other agents selected for comorbidities
Risk reduction Blood pressure and lipid treatment, smoking cessation, kidney and cardiovascular protection
Monitoring A1c, kidney testing, eye exams, foot care, vaccinations and dental health

Weight loss and remission

Substantial sustained weight loss can place type 2 diabetes into remission for some people, especially earlier in the disease. Remission is generally defined by glucose below the diabetes threshold without glucose-lowering medicine for a specified period, but recurrence remains possible and monitoring continues.

How W8MD can help

W8MD physicians and clinicians evaluate weight history, medications, nutrition, body composition, metabolic risk factors and sleep-related barriers before recommending an individualized plan. Available services may include medical weight loss, GLP-1 treatment evaluation, established oral medications, nutrition planning, sleep-apnea care and long-term maintenance support.

When an FDA-approved medication is appropriate and insurance coverage exists, W8MD can review benefits and assist with prior-authorization documentation. Coverage and approval are determined by the health plan.

W8MD can coordinate weight treatment with the patient’s diabetes clinician, choose options with favorable weight and cardiometabolic effects, review hypoglycemia risk, assist with coverage documentation and address sleep apnea. Insulin and sulfonylurea doses may need adjustment as intake and weight change.

Frequently asked questions

Is type 2 diabetes caused only by sugar?

No. Genetics, insulin resistance, beta-cell function, weight, activity and other factors interact.

Can type 2 diabetes require insulin?

Yes. Needing insulin is not a failure and may be temporary or long term.

Can GLP-1 medicine replace all diabetes care?

No. Medication selection, risk-factor treatment and complication screening remain necessary.
