The SELECT cardiovascular-outcomes evidence

Wegovy injection is FDA-approved to reduce the risk of cardiovascular death, nonfatal heart attack and nonfatal stroke in adults with established cardiovascular disease and either obesity or overweight. The approval was based on a large randomized trial of more than 17,600 participants without diabetes.

Major cardiovascular events occurred in 6.5% of participants assigned semaglutide and 8.0% assigned placebo over the study period. This was a relative risk reduction; individual benefit depends on baseline cardiovascular risk and continued standard care.

What the indication means—and does not mean

Established disease

The cardiovascular indication applies to selected adults who already have established cardiovascular disease and overweight or obesity.

Not a substitute

Semaglutide does not replace blood-pressure care, cholesterol treatment, antiplatelet therapy, smoking cessation or emergency evaluation.

Broader improvements

Weight, waist circumference, glucose, blood pressure and inflammatory or lipid markers may improve, but response varies.

Shared decision

Benefits must be weighed against gastrointestinal effects, gallbladder and pancreatic concerns, eye disease in diabetes and other label warnings.
